Metabolic Health & Monitoring

21 Biomarkers

Metabolic health sits at the centre of how your body manages energy, weight, inflammation and long-term disease risk. When the systems that regulate it start to drift — blood sugar creeping up, thyroid function slowing, kidney filtration changing — the effects can be wide-ranging and easy to miss.

This test measures 21 biomarkers across six interconnected systems: blood sugar regulation, thyroid function, kidney health, liver function, cardiovascular risk and key energy nutrients. Not just a snapshot of where you are today, but a baseline to track against — so that every future result tells you something more meaningful.

Results in 2 working days.

What this Metabolic Health & Monitoring measures

21 Biomarkers

Blood Sugar & Inflammation

2

These two markers are at the heart of metabolic health monitoring. HbA1c gives you a three-month average of your blood sugar levels, one of the most reliable early indicators of insulin resistance and type 2 diabetes risk. hs-CRP measures systemic inflammation, which is closely linked to metabolic dysfunction and is now understood to be both a driver and a consequence of poor metabolic health. Together they give you a powerful read on where your metabolism currently stands.

  • HbA1c
  • hs-CRP

Cholesterol (Lipid Profile)

5

Metabolic health and heart health are deeply interconnected. Disrupted blood sugar regulation and insulin resistance directly affect how your body produces and clears cholesterol, making lipid levels one of the most important things to track alongside glucose. This profile goes beyond a single cholesterol number, giving you the full picture, including triglycerides, which are particularly sensitive to diet, blood sugar and metabolic function.

  • Total Cholesterol
  • LDL Cholesterol
  • HDL Cholesterol
  • HDL Ratio
  • Triglycerides

Thyroid Function

2

Your thyroid sits at the centre of your metabolism, controlling the rate at which your body converts food into energy. An underactive thyroid can slow metabolism, contribute to weight gain, raise cholesterol levels and affect blood sugar regulation, all in ways that can mimic or compound metabolic conditions. These two markers together give a clear and reliable picture of thyroid function, ensuring nothing is misattributed or missed.

  • Thyroid Stimulating Hormone (TSH)
  • Free T4 (FT4)

Liver Health

6

The liver plays a central role in metabolic health, regulating blood sugar, processing fats, producing proteins and clearing waste products from the body. Non-alcoholic fatty liver disease, closely linked to insulin resistance and poor metabolic health, is one of the most common yet underdiagnosed conditions in adults today. These six markers reveal how your liver is functioning and whether the demands being placed on it are starting to show.

  • ALT
  • ALP
  • Gamma GT (GGT)
  • Albumin
  • Globulin
  • Total Protein

Kidney Health

4

The kidneys are directly affected by the metabolic conditions this test monitors, poorly managed blood sugar is one of the leading causes of kidney damage over time. Creatinine, urea and eGFR show how efficiently your kidneys are filtering waste from the blood, while uric acid, elevated by diets high in processed food and fructose, is an emerging marker of metabolic syndrome and cardiovascular risk. This panel ensures your kidneys are being monitored as part of the full metabolic picture.

  • Creatinine
  • Urea
  • eGFR
  • Uric Acid

Vitamins & Iron

2

Nutritional status has a direct bearing on metabolic function. Active B12 is essential for energy metabolism at a cellular level and plays a key role in how the body processes homocysteine, a marker linked to cardiovascular and metabolic risk. Ferritin (your body's iron stores) affects energy production, sleep, immune function and cognitive performance. Deficiencies in either can compound the fatigue and reduced capacity that often accompany metabolic imbalances, making them important to track alongside the core metabolic markers.

Why Test?

Metabolic conditions rarely give you warning signs before they take hold. Type 2 diabetes, thyroid dysfunction, early kidney impairment and fatty liver disease can each be present — and progressing — without obvious symptoms. By the time most people notice something is wrong, markers have often been shifting for years.

But this is not just about spotting problems early. It is about building a picture of how your metabolism is working over time, so that the changes you make — to your diet, your activity, your treatment — show up clearly in your results. That is what monitoring really means: not just data, but understanding what is actually working for you.

Frequently asked questions

This is what our customers ask us about the most. For more information, try our help centre.

What does the Men's 40+ Complete Health Check measure?

It measures 32 biomarkers across the areas most relevant to men’s health from 40 onwards: hormones (testosterone and oestradiol), prostate
health (PSA), adrenal function (cortisol), thyroid function (TSH and Free T4), cardiovascular health, liver and kidney function, bone and muscle health, immune health and nutritional status. You receive 6 Healthcoach Scores that translate your results into clear priorities and next steps — a connected picture of your health, not just a list of numbers.

Why is PSA included, and what does an elevated result mean?

PSA (Prostate Specific Antigen) is a protein produced by the prostate gland. Measuring it is the most widely used way to screen for changes in prostate health. An elevated PSA does not mean you have prostate cancer — it can be raised by a number of causes including a benign
enlarged prostate or inflammation. Every set of results is reviewed by one of our doctors. If your result is outside the reference range, we will recommend you discuss it with your GP, who can advise on whether further investigation is needed.

Do I need to collect my sample at a specific time?

Yes. This test includes cortisol, which must be collected within 3 hours of waking. Cortisol follows a daily rhythm and is at its peak in the morning — collecting outside this window will affect your result. We also recommend collecting your sample before eating for the most
consistent cholesterol, blood sugar and hormone results.

Is there anything I should avoid before collecting my sample?

Yes. In the 48 hours before testing, avoid strenuous exercise, ejaculation and any prostate examination — all of these can temporarily elevate your PSA result. Collect your sample in the morning before eating, and make sure you are normally hydrated beforehand.

I take testosterone therapy. Can I still take this test?

Yes, but there are two things to be aware of. First, your results will need to be interpreted in the context of your treatment — exogenous
testosterone affects both total testosterone and oestradiol levels, so please note in your Forth account that you are on TRT so your results can be contextualised appropriately. Second, if you apply testosterone as a gel or cream, do not use the finger-prick collection method. Residual testosterone on the skin can contaminate your sample and skew your result, even after handwashing. Please select the venous blood draw option at checkout.

Do I need to fast before taking this test?

We recommend taking your sample in the morning before eating. This gives you the most consistent results, particularly for your cholesterol,
blood sugar and hormone markers.

What happens if a result comes back outside the normal range?

Every set of results is reviewed by one of our doctors before it reaches you. If anything falls outside the normal range, it will be flagged clearly with a plain-language explanation and a recommended next step. Forth is not a diagnostic service, but your results — reviewed by a clinician and explained in plain language — give you and your GP something specific and concrete to work from.

My GP has not suggested I need any of these tests. Why would I do this?

NHS testing for men in their 40s is typically reactive — triggered by symptoms or specific risk factors. Forth is built for something different: understanding where your health stands before problems appear, not after. A baseline result now gives you and your GP a clear reference point. And the health scores and personalised next steps you receive mean you are never just looking at a list of numbers — you are looking at a picture of your health that you can actually act on.

How often should I retest?

We recommend retesting annually as a minimum. If any markers are borderline — particularly PSA, testosterone, HbA1c or lipids — your GP or
clinician may recommend a shorter interval. That is where the monitoring really earns its value: not one snapshot, but a series of results that show you whether things are improving, holding steady, or starting to shift.

How soon will the test arrive?

If you order your test before midday on a Monday to Friday then your kit will be dispatched the same day.

All our kits are sent out via Royal Mail Tracked24, so it should be with you within 1-2 working days.

How secure is my data?

We have strict processes in place to ensure the protection of your data. Following GDPR the company also operates under tight legal rules about the sharing of data which ensures that data is only shared if it is crucial to the delivery of our service. For example, our doctors see customer results at the time of review, however, after review, access to results is withdrawn.

Learn more about your data security.

Can I have someone take my blood for me?

Yes. We offer two options if you do not want to do our finger prick test. The first is a home appointment where a nurse comes to your home to take a blood sample. The second is to visit a Phlebotomy clinic near you that offers a blood sample service.

A blood sample will be taken from your vein and we will provide you with everything you need to give to the nurse to allow them to take the sample.

The nurse will give you the blood sample to return to us using the pre-paid envelope provided.

Does a doctor review my results?

We have a team of doctors and nurses who look at all results and will comment on any results that are outside of the normal range for your age.

Can I download the results to share with my GP?

Yes, you can download your results from your health dashboard as a PDF to share with your GP.
