What are biomarkers?

Biomarkers are specific compounds we can detect in your blood sample that reflect different things about your health. Your test will tell you your levels for each of the above biomarkers, and whether they are in a normal range.

steps — so you know not just where you stand, but what to actually do about it.

Results land in your Forth dashboard within 2 working days. Retest every 6 to 12 months and you will start to see something more valuable than a single result: how your health is moving over time, and whether what you are doing is working.

What this Foundation Health Check measures

25 Biomarkers

Cholesterol (Lipid Profile)

5

Your cholesterol profile is one of the most important windows into your cardiovascular health — and a single total cholesterol number only tells part of the story. What really matters is the balance between LDL (often called ‘bad’ cholesterol), HDL (‘good’ cholesterol), and triglycerides, a type of fat in the blood that responds directly to diet and lifestyle. Together, these five markers paint a complete picture of how your body is managing fat in the blood and where your heart health stands right now. It’s exactly the kind of detail that makes the difference between knowing your numbers and actually understanding them.

  • Total Cholesterol
  • LDL Cholesterol
  • HDL Cholesterol
  • HDL Ratio
  • Triglycerides

Blood Sugar & Inflammation

3

Blood sugar regulation and inflammation are two of the most powerful drivers of long-term health — and two of the easiest things to miss before they start causing problems. HbA1c gives you a three-month average of your blood sugar levels, offering a far more reliable window into glucose metabolism than a single reading. hs-CRP measures low-grade systemic inflammation, which sits at the root of many chronic conditions, from heart disease to metabolic dysfunction. Uric acid rounds out the picture: elevated levels are closely linked to diet, metabolic stress and cardiovascular risk, making it a useful marker to track alongside the others.

  • HbA1c
  • hs-CRP
  • Uric Acid

Liver Health

6

Your liver is one of the body’s hardest-working organs — filtering toxins, regulating blood sugar, processing fats and producing the proteins your whole system depends on. These six markers reveal how well it’s managing that workload, picking up early signs of strain from diet, alcohol, medication or inflammation while they’re still straightforward to address. Together they offer a thorough picture of liver function, from enzyme activity to protein production, giving you the full story rather than just a headline number.

  • ALT
  • ALP
  • Gamma GT (GGT)
  • Albumin
  • Globulin
  • Total Protein

Full Blood Count

3

Your blood tells a remarkably detailed story about how your body is functioning day to day. Haemoglobin and your red blood cell count show how effectively oxygen is being carried around your body — a core driver of energy, concentration and physical recovery. White blood cell count gives a window into how your immune system is performing and whether your body may be quietly fighting something off. Together these three markers form a foundational read on your overall vitality and resilience.

  • Haemoglobin
  • Red Blood Cell Count (RBC)
  • White Blood Cell Count (WBC)

Vitamins & Iron

3

Deficiencies in these three nutrients are among the most common in the UK — and among the most consequential. Active B12 is essential for energy production, nerve function and the formation of healthy red blood cells. Ferritin measures your iron stores, which affect everything from physical endurance to cognitive performance and immune resilience. Vitamin D supports bone strength, immune function and mood, yet the majority of adults in the UK fall short of optimal levels, particularly through the winter months. Knowing exactly where your levels sit means you can take action based on your biology, not guesswork.

  • Active B12
  • Ferritin
  • Vitamin D (25 OH)

Hormones

2

Oestradiol and testosterone are two of the most influential hormones in the body — for both men and women. Oestradiol shapes mood, bone density, cardiovascular health and cognitive function. Testosterone drives energy, muscle strength, libido and mental resilience. Both play significant and often underappreciated roles regardless of sex, and both shift meaningfully with age, stress and lifestyle changes. Understanding where your levels sit gives you a genuinely useful piece of your full health picture and a baseline you can track over time.

  • Oestradiol (Oestrogen)
  • Testosterone (Total)

Bone & Muscle

3

Strong bones and healthy muscles are built on the right foundations. Calcium and magnesium work in close partnership to support bone density, nerve signalling and muscle function; an imbalance in either can quietly affect everything from energy to recovery. Creatine kinase gives a read on how much physical stress your muscles have been under — useful context whether you train regularly, lead an active lifestyle, or are working through a particularly demanding period. Together, these three markers help ensure the structural side of your health isn’t being overlooked.

  • Corrected Calcium
  • Magnesium (serum)
  • Creatine Kinase

How testing could help you

Getting a blood test result is not the same as understanding your health. Most people who receive results see a number, a reference range, and very little else. No context. No priorities. No clear next step. That gap between data and understanding is exactly what Forth is built to close.

The Foundation Health Check focuses on the markers most linked to the leading causes of preventable ill-health in the UK, including cardiovascular disease, type 2 diabetes, liver disease and nutritional deficiency. Your results do not just tell you where you are today.

They give you a baseline to track against, so that every future test tells you something more meaningful: whether things are improving, holding steady, or starting to shift in a direction worth addressing.
One test is a starting point. Tracking over time is where health intelligence really begins.

SOMEWHAT LOW
Dr Thom Philips, Head of Clinical Services, Forth

Your B12 is in the lower part of the healthy range. B12 works with folate and vitamin C to help the body make new proteins. We always try and promote a food first approach to addressing vitamin and mineral deficiencies so including B12 rich foods into your diet is a good place to start.

Your plan adapts to your body’s needs, considering hormonal changes, age, and treatments for a truly customised approach.

Frequently asked questions

This is what our customers ask us about the most. For more information, try our help centre.

What does the Foundation Health Check test for?

It looks at 25 biomarkers across five areas of your health: your heart and cardiovascular system, liver function, hormones, key nutrients and immune health. You also get 5 health scores that pull your results together and show you what they mean — and what to do next. This is not just a list of numbers. It is a starting point for understanding your health and improving it over time.

Do I need to fast before taking the test?

We recommend taking your sample in the morning before eating. Avoiding food beforehand gives you the most consistent results, particularly for your cholesterol and blood sugar markers.

How long will it take to get my results?

Your results are typically ready within 2 working days of your sample reaching our accredited NHS laboratory. You will get an email notification and can view everything in your secure Forth dashboard — with clear health scores, plain-language explanations for every marker, and personalised next steps.

Can everyone in my family take this test?

Yes, the Foundation Health Check is suitable for any adult aged 18 or over. Each person needs their own kit and their own Forth account. Results are personal, so everyone gets their own dashboard with their own scores and recommendations.

My GP has not suggested I need a blood test. Should I still take one?

NHS blood tests are usually offered in response to a specific symptom or clinical concern. Forth is built for something different: understanding your health before problems appear, not after. A baseline now means that when things do change, you will notice sooner. And the health scores and guidance you receive give you and your GP something concrete to act on, rather than just raw numbers to interpret.

What happens if a result comes back outside the normal range?

Every result comes with a plain-language explanation and a review by one of our Doctors, so you will always understand what you are looking at. If anything falls outside the reference range, we flag it clearly and give you a recommended next step. Forth is not a diagnostic service, but your results give you and your doctor something clear and specific to work from.

I use HRT gel or spray. Is this test suitable for me?

Yes, but you will need to choose the venous blood draw rather than the finger-prick kit. HRT gel and spray applied to the skin can transfer to your fingertips and affect your oestradiol result, even after washing your hands. Please select the venous option at checkout.

I use testosterone gel or cream. Does that affect how I collect my sample?

Yes. Testosterone gel and cream can contaminate a finger-prick sample and skew your result, even after handwashing. If you use topical testosterone, please choose the venous blood draw option at checkout. This applies whether you are using it as part of TRT or as part of an HRT regimen.

Can I track my results over time?

Yes, and this is where Forth really comes into its own. Every result is saved in your dashboard, so each time you retest you can see exactly how your health is changing. Trends tell you far more than a single result ever can. We recommend retesting every 6 to 12 months to build a picture that is genuinely useful over time.

How soon will the test arrive?

If you order your test before midday on a Monday to Friday then your kit will be dispatched the same day.

All our kits are sent out via Royal Mail Tracked24, so it should be with you within 1-2 working days.

How secure is my data?

We have strict processes in place to ensure the protection of your data. Following GDPR the company also operates under tight legal rules about the sharing of data which ensures that data is only shared if it is crucial to the delivery of our service. For example, our doctors see customer results at the time of review, however, after review, access to results is withdrawn.

Learn more about your data security.

Can I have someone take my blood for me?

Yes. We offer two options if you do not want to do our finger prick test. The first is a home appointment where a nurse comes to your home to take a blood sample. The second is to visit a Phlebotomy clinic near you that offers a blood sample service.

A blood sample will be taken from your vein and we will provide you with everything you need to give to the nurse to allow them to take the sample.

The nurse will give you the blood sample to return to us using the pre-paid envelope provided.

Does a doctor review my results?

We have a team of doctors and nurses who look at all results and will comment on any results that are outside of the normal range for your age.

Can I download the results to share with my GP?

Yes, you can download your results from your health dashboard as a PDF to share with your GP.

Learn how to export your results.
